BLUE CARD FORMS
All over 18 Active Members are required to hold a valid Blue Card. If you do not, please print out the applicable application form below, complete and return to the club for submission. You will need to bring in your Drivers License and 1 other form of ID. You can return this form at any time to reception.

Volunteer New & Renewal Form
(For a new or renewal blue card application)

Volunteer - Authorisation to Confirm a Valid Blue Card Form
(If you already hold a blue card through another organisation other than Surf Life Saving)

(For Police Officers & Teachers only)

2011/12 Proficiency Details

• This season proficiencies will be run on patrol over the first 3 weekends of the patrolling season.

• It's very important that you attend your rostered patrol so you don't miss out.

• You will still need to complete your pool swim with a pool coach (swim sign off sheets below) or at one of the dates set by the club. SUN 4 SEP 9am @ TSS POOL.

• If you are a long service member or are not on a rostered patrol you can attend any one of the patrols over the first 3 weeks of the patrolling season.

• IRB crew & driver proficiencies will be run separately. (see below)

• This season IRB Driver and Crew proficiencies will be run separately on the following dates.
  
  SAT 10 SEPTEMBER @ 8am or SUN 11 SEPTEMBER @ 8am
  SAT  8 OCTOBER   @ 8am or SAT 12 NOVEMBER  @ 8am 

• It's very important that you attend one of these proficiencies as we need as many crew and drivers proficient for the season ahead.If you missed out completing last season please make the effort this year.
